Service Settings

Host Home (Shared Living)

A Host Home is a private family home that welcomes an individual to live as part of the household.

What this offers:

• Consistent support in a neighborhood home
• Daily living skill-building (meal prep, hygiene, budgeting, chores)
• Community integration (work, volunteering, recreation, libraries, gyms)
• Transportation to appointments and activities (per plan)
• Medication administration and health coordination
• Behavior supports and positive strategies
• 24/7 on-call support

Best for: People who thrive with a stable caregiver relationship and a quieter, family-style environment.

Group Home (24/7 Staffed)

A Group Home provides care in a small, licensed residence with trained staff on site.

What this offers:

• 24/7 staffing with individualized support plans
• Structured routines while honoring personal choice
• Daily living and social skill development
• Community activities, recreation, and employment supports (per plan)
• Medication administration and health coordination
• Safety systems, drills, and on-call management

Best for: People who benefit from multiple staff available, more structure, and peers in the home.
